The Clinical Dietitian Service is for people diagnosed with or at high risk of developing a long-term condition. Dieticians work with people to help them better understand how what we eat affects our health and wellbeing.
The service provides a programme of up to six weeks including:
- an initial clinical assessment,
- provision of a nutrition care plan including goal setting and if appropriate meal planning,
- education on food selection and label reading,
- specific nutrition requirements for long term conditions.
This service is provided free of charge and is delivered at a location convenient for the patient.
