Clinical Pharmacists/Primary Care Support Pharmacists
General practice based clinical pharmacists work alongside the general practice team to provide medicines related advice and support to clinicians as well as patients – particularly those with long term conditions and/or older persons.
The clinical pharmacist role is to:
• Improve medicine related health outcomes for patients so that ‘the right people receive the right medicines at the right time’
• Improve medicines safety
• Reduce unnecessary and inappropriate use of medicines
• Reduce wastage of medicines
The clinical pharmacists will only receive referrals from members of the general practice team and other clinicians such as hospital specialists, specialist nursing teams, Central PHO clinicians, community and/or hospital pharmacists.
