Community Cardiac Rehabilitation Programme

 

The Cardiac Rehabilitation Programme is an education programme that empowers positive lifestyle behaviours to get people back to living as full a life as possible following a heart event. The programmes seven education sessions include: 

  • managing mixed emotions, 
  • nutrition, 
  • understanding heart disease, 
  • understanding medications, 
  • emergency at home and CPR instructions, 
  • exercise guidelines, 
  • coping strategies. 

Cardiac Rehabilitation can help prevent future heart problems, improve quality of life and help with overall wellbeing. The programs are delivered across the region and there is no cost to attend.
People are referred into the program by their GP, the hospital they have been in or they can refer themselves by calling Central PHO.
